Value Added Services

As a member, you are able to get extra services in addition to your regular benefits. These are called Value Added Services (VAS). For questions or to learn how to get these services, please contact Member Services at 1-877-236-1341 (TTY: 711).

Whole Health Transportation

Transportation to Value Added Services (Weight Watchers, GED Testing, etc.) as well as additional services such as rides to the pharmacy to pick up prescriptions or home delivery of prescriptions where available.

• 2 round trips per month to the hospital for parents/caregivers

Healthy Weight Program

A membership for the YMCA or $250 per calendar year for summer camps and sports teams offered through the YMCA.

• Must be an active user (6 times over two consecutive months) 

• Must be enrolled with Delaware First Health 

Diabetes Prevention Program

Comprehensive Diabetes Prevention Program 

• Members at high risk for diabetes, including those with pre-diabetes diagnosis

GED Testing

GED Testing vouchers and tutoring

• 16 or older and not currently enrolled in school

Member Incentives

Earn dollar rewards for making healthy choices

• Can be used on items such as utilities, transportation, daycare, etc.

• Certain items are excluded such as tobacco & alcohol.  Cards must be used in person.

SafeLink and ConnectionsPlus Phone

Assistance with obtaining a free cell phone through SafeLink or through our ConnectionsPlus program.

• One phone per household, 4.5G of data per month, 350 minutes of talk time

• ConnectionsPlus enrolled through Care Coordinator only

Vision Benefit

 

Every two years adults are eligible for a comprehensive eye exam with refraction and an eyewear allowance of up to $160 • Must be 21 years or older and must not exceed $160 for eyewear. Eyewear includes for frames and lenses, lens upgrades, or contact lenses. Limited to every two years.

Housing

 

One-time housing transition grant of up to $2,500 for members who are experiencing homelessness to supportive or independent housing; or members who are transitioning from nursing facilities to community living (in addition to state’s benefit)

• Foster Care must be 18-26

• Must have assigned Care Manager and cannot exceed $2,500 per lifetime

• Funds may be used for security deposits, rent, utilities, support animal deposits, food, household goods,  eviction diversion services

Tutoring

Up to $200 per year in tutoring vouchers for members k-12 grade who are at risk of falling behind on one or more core subject area

• Must be ages 5-18 and proof of failing grades may be required

Home-Based Asthma Interventions

At home Asthma Support including $250 per year for non-clinical, home-based needs such as mold removal, carpet cleaning, hypoallergenic bedding, low VOC cleaning products, air purifiers, and pest control

• Must have asthma diagnosis and cannot exceed $250 annually

Post Discharge Meals

 

Post Discharge meal delivery benefits to members at high risk for re-admissions. Eligible members can receive 3 meals a day for 7 days after being discharged from the hospital or other inpatient facility. • Maximum of 42 meals per year, must be 18 years or older and in Case Management

Pharmacy OTC

Members may select from an Enhanced list of Over The Counter (OTC) items

• Up to a maximum of $30 per calendar quarter, per household.

MyStrength App for BH Support

Access to MyStrength mobile app to help manage stress, anxiety, chronic pain & more

• Must be 13 years or older

• 4 round trips per month for 60 days post-discharge from a facility to attend AA/NA/CA related meetings or events

Practice Dental Visits 

Adult practice dental visits when they establish with a new dentist to meet with the dental team, voice preferences and concerns, and understand what happens in a dental appointment in advance of any exams or treatments

• Must be 21 years or older with IDD

Social Isolation Support

Membership to local senior centers and classes for social, recreational, and education activities. Virtual social isolation support. Members also receive a social isolation toolkit with education materials.

• Must be 60 years or older and cannot exceed $250 annually
