Every year, millions of people of all ages use and rely on LTSS, from meal delivery to self-directed personal care services.

At Delaware First Health, we’re here to support you with Long Term Services and Supports (LTSS).

What is LTSS? It’s a program for those who need a more hands-on, nimble, and custom care plan. LTSS gives our members more control over their health and the care they receive.

The Value of LTSS

Here are some of the ways LTSS takes care of members:

Free to everyone enrolled in LTSS, a care manager helps members manage and create their care plan. They are also the go-to for any questions they might have about their benefits.

An attendant care aid is someone who helps with tasks like meal prep, cleaning, bathing, and getting dressed.

Adult day services offer trained staff for health services and ways for members to be social.

Allows members to be in a 24/7 facility that provides a full range of care.

Respite care gives unpaid caregivers a short break while someone else supports their loved one at home or in a facility.

Gives members apartment style living and makes personal care and other services, like meal delivery, available.

Gives members the ability to choose who gives their care and how they get it.

Meals are delivered right to member's door. They then don’t have to worry about shopping for food.

Rides to planned medical appointments.

A device calls for help at the press of a button.

Qualifying & Signing Up

How do you qualify and enroll for LTSS?

Go to Delaware Assist Portal to apply for Medicaid and/or Long Term Care Medicaid to learn more.

Selecting a MCO

If you receive a letter from Division of Social Services or Division of Medicaid and Medical Assistance telling you are eligible for Medicaid, you should follow the instructions in the letter.

To select your Managed Care Organization, call the Health Benefits Manager at 1-800-996-9969 to make your choice.

Using Your Plan

Once you’re approved and enrolled in your plan you will:
