We are excited that you selected our provider network as your network of choice. In addition to joining the Delaware First Health network, you may also use the Contract Request Form to apply for participation in our Wellcare (Medicare) and Ambetter Health of Delaware (Marketplace) networks.
If you are a non-contracted provider or are a current provider who wishes to add a line of business or service(s) to your existing contract, please complete the Contract Request Form.

For Contracted Providers
As a contracted provider, you can rely on:
- A comprehensive approach to care for your patients through disease management programs, healthy behavior incentives and 24-hour toll-free access to registered nurses
- Initial and ongoing provider education through orientations, office visits, training and updates
- A dedicated claims team to ensure prompt payment
- Minimal referral requirements
- A dedicated provider relations team to keep you informed and maintain support in person, by email or by phone
- The ability to check member eligibility, authorization and claims status online
- Healthcare collateral for your patients (e.g., information about our benefits and services) and educational displays for your office
Provider Data Updates
Delaware First Health needs your help to ensure we have your most up to date information about your provider data. We have created an easy way for you to request updates to your information and ensure we receive what we need to complete your request in a timely manner. Please visit our Provider Data Updates webpage to do any of the following:
- Make updates to your demographic information.
- Add facilities to your existing contract. This includes Ancillary, Behavioral Health, and Hospital providers. The page includes links and information on the documents required to submit your request.
- Add practitioners to your existing contract.
