Our Find a Provider tool makes it easy for patients, members, and referring clinicians to discover and connect with you. By showcasing accurate, up‑to‑date information about your practice—including specialties, locations, and accepted plans—the tool helps ensure patients can quickly find the right care at the right time.

As part of our provider network, your participation increases your visibility, supports appropriate referrals, and helps patients make informed decisions with confidence. To ensure patients see the most accurate information, providers are encouraged to regularly review and update their details using our Provider Data Update Forms, which are the primary way to submit changes to practice, location, or contact information.

Keeping your profile current strengthens access to care and improves the overall patient experience across our network.

DFH uses the Preferred Drug List (PDL)/Formulary as developed by the Delaware Medicaid Program. Please use the PDL/Formulary to find more information on the drugs that are covered.
