We’ve gathered a comprehensive collection of resources to make your work easier and more efficient, so you can stay informed and stay focused on what matters most—providing excellent care.
Delaware First Health is pleased to introduce two affiliated products designed to meet healthcare needs in Delaware. With the addition of Ambetter Health of Delaware (Health Insurance Marketplace), we can provide affordable, comprehensive solutions for lower-income individuals and families who may not qualify for Medicaid or have access to employer coverage. And, by adding Wellcare (Medicare/DSNP), we can better coordinate care for our DSNP members.
Beginning January 1, 2026, we will launch an aligned Dual Special Needs Plan (D-SNP), Wellcare by Delaware First Health. This new plan is designed for individuals who qualify for both Medicare and Medicaid, allowing their benefits to be coordinated and managed by a singlehealthcare organization.
All plans are designed to serve Delawareans as their life situation evolves.

Delaware First Health is committed to connecting our members to resources helping our members find resources in the community. We have developed strategic partnerships with community-based organizations to ensure our members have access to receiving services. Through our FindHelp website, members can search for free or reduced cost services like food pantries, job training, financial assistance and more.

At Delaware First Health, we work to improve not only the health of our members, but also the economic and social issues that can act as barriers to proper care. We recognize that in addition to accessing the quality healthcare you provided by our network, social determinants of health (SDoH) also impact patient outcomes.
We are proud to offer extra services to keep our members healthy by enhancing our programs and community connections. We invite all providers to encourage members to take advantage of our value added benefits and wellness programs.
Some of our value-added benefits include:
Blood Pressure Self-Monitoring Program - 16-week program to help adults with high blood pressure or hypertension lower and manage their blood pressure.
- For qualifying members ages 18+ with a diagnosis of high blood pressure, hypertension or taking antihypertensive medications.
- This program is available virtually or in person at participating Delaware YMCAs.
- Providers may complete the YMCA of DE Community Health Program Interest Form to refer DFH members to this value-added benefit.
Diabetes Prevention Program (DPP) - A lifestyle change program focused on healthy eating and physical activity for members at high risk for diabetes
- Medicaid members age 18+ who are not currently pregnant, have not been previously diagnosed with diabetes, and have a Body Mass Index (BMI) of 25 or higher (23 or higher if Asian American) can be referred to the program
- DFH partners with two organizations to offer this program. Members should choose the program that works best for them:
- YMCA of Delaware - offering online and in-person classes across the state at YMCAs and additional locations. Spanish language classes are available in-person and in Sussex and New Castle counties. If you join the DPP through the YMCA, you also get a 4-month YMCA membership.
- Providers may access the YMCA of DE Community Health Program Interest Form to refer members to the DPP value-added benefit with the YMCA
- Health Promotion Council - offers classes online and in-person at Bebee Healthcare (Sussex Co) and Focus Pharmacy (Smyrna). Spanish language classes are available online.
- Providers can fill out this form to refer a member to the Diabetes Prevention Program through the Health Promotion Council referral form.
- YMCA of Delaware - offering online and in-person classes across the state at YMCAs and additional locations. Spanish language classes are available in-person and in Sussex and New Castle counties. If you join the DPP through the YMCA, you also get a 4-month YMCA membership.
Community-Based Wellness Programs - DFH members stay healthy with our partner organizations. Members may choose one organization to participate with each year.
- Boys & Girls Clubs (members age 5-18)
- $250 toward memberships and programs
- YMCA (members age 5+)
- Individual, 12-month membership and programs
- Requires annual wellness visit
- LEAN Life-long Essentials for Activity & Nutrition Program (members age 18+ with BMI of 25 or higher)
- 12 weekly, 90-minute in-person sessions that include guided discussions on healthy eating and more.
- Small-group personal training workouts
- 4 month YMCA membership
- Providers may access the YMCA of DE Community Health Program Interest Form form to refer members to the LEAN Program value-added benefit.
- Senior Center (members age 60+)
- $250 toward membership and programs at partner senior centers.
My Health Pays® Activity Rewards - Members earn rewards when they complete healthy activities like yearly wellness visits, flu shots, and cancer screenings. You may review all DFH member rewards on the My Health Pays® Rewards page.
Home-Based Asthma Interventions - Members with an asthma diagnosis and the required provider form, get up to $250 per year in home-based, non-clinical asthma supports:
- Hypoallergenic bedding
- Air Purifiers
- Pest Control
Providers may complete the Home-Based Asthma Interventions VAB Form to refer DFH members to this value-added benefit.
