Ensuring Continuity of Care for D-SNP Members

Delaware is moving to Exclusively Aligned Enrollment (EAE) for Dual Eligible Special Needs Plans (D-SNPs), effective Jan. 1, 2026. D-SNP members will be aligned to the same Medicaid Managed Care Organization (MCO) for improved coordination of care. Continuity of Care (CoC) protections will apply to these members who are in an active course of treatment or have existing authorizations, should alignment or plan changes occur.
How Providers Should Proceed: Maintain Care and Verify Coverage
1. Do not interrupt care for D-SNP–Medicaid aligned members who are in active treatment or who have existing authorizations with their previous MCO. Continue services during the CoC period.
2. Verify eligibility and alignment via your normal eligibility channels.
