Delaware First Health, following NCQA guidelines, will annually gather Healthcare Effectiveness Data and Information Set (HEDIS) data from our healthcare providers, a vital project that assesses specific aspects of care and service. We intend to request medical records in the upcoming months and wish to inform you in advance.

Please note that the requested address is the address where your organization’s medical records requests are received.
