Reminder: DHIN C-CDA Documents and HEDIS

Please see the attached communication regarding Acceptable Documentation for Healthcare Effectiveness Data and Information Set (HEDIS) Reporting. The letter explains that documentation must be submitted directly from the provider’s Electronic Health Records (EHR) system of record and that Consolidated Clinical Document Architecture (C‑CDA) and HIE data are considered supplemental only. Reviewing this information will help prevent delays and rework during HEDIS abstraction.
