Delaware First Health and the Centene Foundation Announce Grant Program Open Submission Period

Delaware First Health (DFH) and the Centene Foundation are now accepting grant applications from Delaware-based 501(c)(3) nonprofits and providers that serve the Medicaid population and address social drivers of health—such as housing instability, food insecurity, and transportation access.
“We are pleased to offer financial support for organizations that address social drivers of health and make a difference in communities throughout Delaware,” said Bill Wilson, DFH President and CEO. “People are more likely to schedule a doctor's appointment when they aren't worried about the roof over their heads, feeding their families or getting to work.”
Grant Details:
- Total funding: $500,000 over 3 years
- Minimum grant: $10,000
- Application deadline: June 13, 2025
- Awards announced: July 2025
- Priority given to projects focused on housing or transportation
Eligibility Requirements:
- Must serve the Medicaid population
- Must be a Delaware-based 501(c)(3)
- Must show measurable outcomes over time
- Must submit all required documents (program materials, W-9, 990, IRS letter, etc.)
