Pregnancy-Related Rewards

• $20 Complete Notification of Pregnancy Form Once per year.

• $50 Prenatal Visit within First Trimester or 42 Days of Enrollment Once per pregnancy.

• $20 For 3 Prenatal Visits Requires three prenatal visits to receive $20 reward. Earn a $20 reward after your third and sixth prenatal visits.

• $15 Prenatal Tdap Vaccine One per pregnancy.

• $15 Prenatal RSV Vaccine One per pregnancy.

• $10 Low-Dose Aspirin Fills for Prescribed Members. Can be earned up to four times per pregnancy.

• $40 Postpartum Visit One per pregnancy. Must be completed 7-84 days after delivery.

• $20 Flu Vaccine Ages 6 months and up. Once per year.

• $15 Infant Rotavirus Vaccine Series One-time Reward.

• $50 for 3 Infant Well Visits Ages 0-15 months. Up to two rewards per lifetime. Requires three visits or claims to receive $50 reward.

• $25 Infant Well Visit Ages 15-30 months. Up to two rewards per lifetime.

• $25 Child Lead Screenings

    o Ages 9-18 months: One reward.

    o Ages 19-27 months: One reward.

• $25 Child Well Visit Ages 3-18. Once per year.

• $15 Adolescent HPV Vaccine Ages 9-13. One-time reward.

• $25 Asthma Controller Med Fill Up to six rewards per calendar year.

Your My Health Pays reward dollars are added to your rewards card after we process the claim for each activity you complete. If you are earning your first reward, your My Health Pays™ Visa® Prepaid Card will be mailed to you in an unmarked envelope.

What if I have not received my My Health Pays™ rewards card?

Once you complete a qualifying healthy activity, we are notified, and your card will be mailed to you. You will receive your card approximately three months after you complete your first qualifying activity. Remember to keep your card, as future reward dollars will be added to it for each qualifying healthy activity you complete. If you have questions, please contact us at 1-877-236-1341 (TTY: 711).

How do I activate my My Health Pays™ rewards card?

Once you receive your My Health Pays™ rewards card in the mail, please follow the instructions on the activation sticker. You will need to have your card with you to complete activation.

Where can I check my My Health Pays™ rewards balance?

To check your balance, login to your Secure Portal account.

When will my reward be added to my card?

Your rewards will be added to your card approximately two months after you complete the qualifying activity. You will receive your card with your first reward approximately three months after you complete your first qualifying activity.

When do my My Health Pays™ rewards expire?

Funds expire 90 days after termination of coverage or 365 days after date reward was earned, whichever comes first.

What if my card is not working at Walmart?

Be sure your card is activated and check to make sure you have funds on the card. Call Member Services at 1-877-236-1341 (TTY: 711) if you have any questions.

Can I use my card at Walmart.com?

No. Your card must be used in store at Walmart.

Can I purchase gas with my card?

No. Your card can be used to purchase transportation such as bus passes.
