2026 SENIOR HEALTHY FOOD BENEFIT
$150 per year to support access to selected healthy foods for members age 65+.
• For members age 65+
• Sign up for this benefit and place orders online, via the Benefits Pro app by Nations Benefits, or over the phone at 1-877-407-0305.
2026 BLOOD PRESSURE SELF-MONITORING PROGRAM
16-week program to help adults with high blood pressure or hypertension lower and manage their blood pressure.
• For qualifying members ages 18+ with a diagnosis of high blood pressure or hypertension or who are taking antihypertensive medications.
• This program is available virtually or in person at participating Delaware YMCAs.
• Fill out this form to learn more about this program and enroll through the YMCA.
2026 $0 PHARMACY CO-PAYS FOR DUAL MEMBERS
• For members with Medicaid through Delaware First Health and Medicare through Wellcare.
• No sign up required. Eligible members are automatically enrolled.
Earn rewards when you complete healthy activities like a yearly wellness visit, flu shot, and cancer screenings. See all rewards here.
• No signup required. Your card will be mailed to you after you complete your first qualifying activity.
• Be sure to activate your card.
• Additional rewards will be added to your card, so please keep your card.
• You can use the card:
o In-person at Walmart or Sam’s Club
o For utilities, transportation, phone bills, childcare, education, or rent.
• Excludes alcohol, tobacco, and firearms.
PHARMACY OVER THE COUNTER (OTC) CATALOG
$120 per year per household to spend on over-the-counter (OTC) products (English PDF) (Spanish PDF) like diapers, cold remedies, shampoo, period products, and more.
• You get $30 every 3 months. If you don’t use it, it goes away.
• Order by phone 1-888-262-6298, online, or in the OTC Health Solutions app (App Store (iOS) or Google Play (Android) ).
• No prescription needed.
PRACTICE DENTAL VISITS
Visit the dentist before your exam to meet the dental staff, ask questions, and feel more comfortable.
• For members:
o All kids age 20 and younger.
o Adults age 21+ with an intellectual or developmental disability (IDD).
• Up to 4 practice visits per lifetime.
• Cannot be used on the same day as a dental appointment.
COMMUNITY-BASED WELLNESS PROGRAMS
Get up to $250 per year to stay healthy with our partner organizations.
Members can choose one organization per year.
• Kids (ages 5-18): Boys & Girls Clubs.
• Members (age 5+): YMCA. Requires annual Wellness Visit.
• Adults (age 18+ with BMI of 25 or higher): LEAN (Lifelong Essentials for Activity & Nutrition Program). 12 weekly, 90-minute in-person sessions that include guided discussions on healthy eating and more, as well as small-group personal training workouts. Includes 4-month YMCA membership. Have your healthcare provider submit this form to enroll.
• Adults (60+): Senior center membership and programs. Offered at Modern Maturity Senior Center in Dover.

ADULT VISION SERVICES
Get a free eye exam and up to $160 for glasses or contacts every year.
• For members age 21+.
• $160 for eyewear includes frames, lenses, lens upgrades, or contact lenses.

HOUSING
Up to $2500 in housing resources.
• Available to eligible members who are:
o Homeless or
o Moving from a facility* or foster care (ages 18-26) to independent living.
• Must have a Case Manager or Care Coordinator.
• Can be used to help pay for security deposits, rent, utilities, food, household goods, eviction help.
• Cannot exceed $2500 per lifetime.
• Contact your Care Coordinator or Case Manager for more information.
*This is in addition to State allowance for LTSS members.
HOME-BASED ASTHMA INTERVENTION
Get up to $250 per year in home-based asthma support.
• For members with an asthma diagnosis.
• For non-clinical, in-home supports such as:
o Hypoallergenic bedding
o Air purifiers
o Pest control
• Your healthcare provider can submit this form to enroll you in the program.
POST-DISCHARGE HOME MEAL DELIVERY
Members leaving the hospital or another inpatient facility who need assistance with food can qualify to receive post-discharge home meal delivery.
• For members 18 or older not in LTSS.
• 3 meals a day for 7 days.
• Meals must be ordered within two weeks of discharge.
• Up to 42 meals per year.
TUTORING FOR KIDS
Is your child at risk of falling behind in school? Get up to $200 per year for tutoring with our online tutoring partner.
• For members ages 5-18 in school (grades K-12).
• Tutoring is only available online through Delaware First Health’s approved tutoring partner.
• You may need to show proof of failing grades.
• Up to $200 per year (6 hours of tutoring).
GED TESTING & TUTORING
Get up to $200 per year to help you earn your GED.
• For members age 16+ who are not in school.
• Must use Delaware First Health’s approved GED partner for testing or tutoring services.
• Up to $200 per year.
WHOLE HEALTH TRANSPORTATION
Delaware First Health gives rides through our Whole Health Transportation Value Added Benefit.
• To book a Whole Health Transportation ride, contact your Care Coordinator or Case Manager, call Member Services at 1-877-236-1341 (TTY: 711), or call 1-877-659-8416.
• Book rides at least 3 days in advance.
Get Whole Health Transportation rides to:
• 4 roundtrip rides per month for health-related social needs.
• 2 roundtrip rides per month to the hospital for member’s parents or caregivers
• 4 rides per month for 60 days after leaving a hospital or inpatient/residential facility to go to community recovery groups (like AA or NA).
• Other Delaware First Health Value-Added Benefits (like GED tests).
• Delaware First Health events (like Member Advisory Council meetings).

Spanish-language classes are available online and in person in Sussex and New Castle Counties.
If you join the Diabetes Prevention Program through the YMCA, you also get a 4-month YMCA membership.
